Nobody should have to pay someone to fill in a form

Templates we use at our own desks, published so that nobody has to pay anyone to fill in a form. All of these are free, may be reproduced, and are revised when the underlying rules change.

Templates

8 files across 6 categories

Each carries the date it was last revised. Where a file is still being prepared, the card says so and offers a way to ask for it rather than a button that goes nowhere.

Using these templates

A template is a starting point, not a substitute for thinking about your own facts. An RTI application copied without adjusting the question will be refused as cleanly as one written badly from scratch. If you want help adapting one, the clinic exists for exactly that.

RTI assistance

Copy them freely

Print them, translate them, put your own organisation’s name on the covering note. We ask only that the passages setting out limits are carried across intact — they are the parts most often dropped, and the parts that protect the person using the form.

Work with us

Something missing

If there is a form you keep needing and we do not have it, say so

Most of what is on this page exists because somebody asked for it at a desk and we realised we were explaining the same thing every week.
